Key points

  • Four age ranges: 18–25, 45–50, 60–65 and 70–75.

  • A 30–45 minute discussion with a healthcare professional.

  • One fully covered assessment per age range, with no upfront payment.

What is this appointment for?

To take time to discuss your health and habits. Together, you choose the subjects that matter to you and a few realistic goals.

Topics may include sleep, diet, exercise, tobacco or alcohol, mental or sexual health, vaccinations and screening.

What happens?

  1. You review your situation and what you would like to improve.

  2. Together, you choose your priorities and identify difficulties to take into account.

  3. You leave with a personalised plan: practical actions and, if needed, professionals to contact.

Before booking

Must I complete a questionnaire?

A self-assessment questionnaire can help prepare the discussion, but it is not compulsory.

Is this a blood test or a full examination?

It is primarily a preventive health discussion. It does not include routine blood tests and does not replace a consultation about a symptom.

Can I do this with my nurse?

Yes. Nurses are among the professionals who can provide it. Contact the practice to arrange a dedicated appointment and check your age range.

Preparing your questions

Write down two or three topics you want to discuss. Bring vaccination and screening information if you have it.
