After a hospital stay
New patients can contact the practice after hospital discharge, with or without technical nursing needs. An initial discussion, which may be by telephone, helps organise care. Tell us your discharge date, prescriptions and chosen pharmacy or equipment provider so we can coordinate with them.
You are free to choose your nursing practice, doctor, pharmacy and medical equipment provider.
Home visit area
The area includes Buffon, Montbard, Aisy-sur-Armançon, Benoisey, Crépand, Montigny-Montfort and its hamlet Villiers, Quincerot, Quincy-le-Vicomte, Rougemont, Saint-Rémy and Touillon. Contact the practice to arrange a visit at your address.

Vaccinations and preventive health assessments. Antoine GRAVIER is trained to prescribe and administer vaccines from age 11 within the French vaccination schedule and regulatory exceptions. Under age 11, injections are arranged on medical prescription.

End-of-life and comfort care, working with the medical team and relatives according to the patient’s wishes. Nursing care for prescribed pain treatments, PCA pumps and syringe drivers.

Enteral and parenteral nutrition, stoma care and urinary catheter care. Equipment is organised with the patient’s chosen pharmacy or provider.

Working with your other care providers
The practice works with hospital-at-home services (HAD), home nursing services (SSIAD), mobile palliative care teams (EMSP) and DAC21.
It provides care in Côte-d’Or and Yonne. The HAD service and mobile palliative care team involved depend in particular on the patient’s municipality.
The practice is a member of CPTS Auxois Morvan.

Patients staying temporarily
You can also contact us for nursing care during your stay. Call in advance to discuss your prescription, documents and appointment availability.
Antoine speaks some English. Professional telephone interpreting can be arranged through the URPS, under the ARS-funded scheme. Please tell us which language you need when you first contact us.
